Here are the details you will need:
Advise them that you would like to explore a directed kidney donation for Krista Merkley
They will ask for my Nephrologist’s name: Dr. Andrew House - LHSC
You will be asked to provide an email address where they can send you information and forms
Please note that you will not be considered as a potential donor until you return the completed forms to them.
As a reminder, you can change your mind at any time and no information is shared with me unless you choose to do so. To begin the process, you simply need to call the number and they will send you some helpful donor information as well as a medical questionnaire to fill out.
